Manage clinical documentation accurately following medical terminology standards and electronic health records (EHR) management protocols. Provide patient education on injury prevention, rehabilitation exercises, and home care strategies to promote recovery and independence. Perform discharge planning to ensure seamless transition from inpatient or outpatient settings to home or community care environments. Utilize specialized techniques such as trigger point therapy and physiotherapy modalities to enhance treatment effectiveness. Work with diverse populations including geriatrics, pediatrics, neurology patients, and those requiring outpatient or inpatient rehabilitation services. Maintain compliance with clinical documentation standards and participate in ongoing professional development related to physiotherapy practices. Qualifications Valid licensure or registration as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within acute care, inpatient hospital settings, outpatient clinics, or home health environments. Strong knowledge of physiology, anatomy, orthopedics, neurology, and geriatrics to inform effective treatment strategies. Familiarity with medical terminology and clinical documentation standards essential for accurate record-keeping. Experience working with electronic health records (EHR) systems or electronic medical records (EMR) systems for documentation purposes. Demonstrated ability to develop individualized care plans based on patient assessments and clinical findings. Excellent client communication skills to educate patients effectively and foster trust throughout the rehabilitation process. Ability to work collaboratively with multidisciplinary healthcare teams in outpatient rehabilitation centers or hospital settings. Knowledge of discharge planning procedures and experience working with geriatric patients preferred.
