Manage clinical documentation standards by accurately recording patient progress, treatment details, discharge planning notes, and adhering to electronic health records (EHR) management protocols. Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care plans and optimize patient outcomes. Conduct discharge planning to ensure seamless transition from hospital or clinic to home or community settings with appropriate follow-up care instructions. Engage with patients and their families through clear communication about treatment goals, progress updates, and education on physiotherapy techniques for ongoing self-care. Skills Proven experience practicing in a physical therapy role within inpatient hospital settings or outpatient clinics. Strong knowledge of anatomy and physiology principles relevant to orthopedics, neurology, pediatrics, geriatrics, and home health care environments. Familiarity with outpatient rehabilitation center operations and outpatient experience to deliver effective therapy sessions. Expertise in medical terminology and clinical documentation standards necessary for accurate record keeping. Skilled in discharge planning processes for diverse patient populations across acute care and community settings. Proficient in electronic health records (EHR) systems or EMR systems for efficient documentation and data management.
