Job Description

Tender Touch Rehab Services is seeking a flexible Physical Therapist Floater to provide on-site coverage across skilled nursing and long-term care facilities. In this role, you'll deliver individualized, evidence-based therapy to seniors, focusing on function, safety, and quality of life. Responsibilities include evaluations, treatment planning, documentation, and close collaboration with an interdisciplinary team. Responsibilities Provide on-site PT coverage across assigned post-acute and skilled nursing facilities as a floater Evaluate senior patients, develop individualized, evidence-based treatment plans, and update goals Deliver therapeutic interventions to improve mobility, strength, balance, function, and safety Document assessments, treatments, and progress in EMR in accordance with regulatory and company standards Collaborate with an interdisciplinary team, including nursing and rehab staff, to optimize patient outcomes Educate patients, families, and caregivers on home exercise programs, safety, and functional strategies Adapt quickly to different facility workflows, teams, and patient populations while maintaining clinical excellence Contribute to a supportive, patient-centered culture that emphasizes dignity, compassion, and respect for seniors Required Skills Geriatric physical therapy Skilled nursing/long-term care rehab Functional mobility and gait training Therapeutic exercise and neuromuscular re-ed Balance and fall prevention Wound care and pain management basics Care planning and documentation (EMR) Interdisciplinary team collaboration Patient and family education Time management and caseload prioritization
