Responsibilities:
Evaluate and assess patients' functional status and mobility. Develop individualized treatment plans to maximize independence and safety. Provide skilled therapeutic interventions in patients' homes. Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety. Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals. Complete timely electronic documentation while maintaining the highest standards of patient care.
Qualifications:
Graduate of an accredited Doctor of Physical Therapy (DPT) program. Licensed or license-eligible Physical Therapist in the state of practice. New graduates are strongly encouraged to apply! Excellent communication and interpersonal skills. Valid driver's license and reliable transportation.

Base Pay , depending upon qualifications, status, territory, market alignment and subject to negotiation: $55.00 - $65.00 per point ($85,800 - $101,400 annual earning potential) As an accredited, regulated, certified, and licensed home health care provider, BAYADA complies with all state/local mandates.
