We are currently recruiting for a compassionate Full Time - Physical Therapist (PT) to reliably serve adult and geriatric home health patients throughout Gloucester County. Why Choose Home Health? Build meaningful relationships through one-on-one patient care Create your own schedule and enjoy greater work-life balance Focus on quality care without the productivity pressures of a clinic Work independently while being supported by a collaborative clinical team Help patients achieve their goals in the environment where they are most comfortable—their home What You'll Do Perform home visits in designated geographic territories Complete comprehensive physical therapy evaluations Assess mobility, strength, balance, gait, transfers, endurance, and safety risks Develop individualized treatment plans to improve function and independence Provide therapeutic exercise, gait training, balance training, and other evidence-based interventions Collaborate with nurses and other clinicians as part of a multidisciplinary care team Educate patients and caregivers on exercises, safety, and disease management Document patient care using a user-friendly electronic medical record system What you should have: Active and current license to practice as a professional Physical Therapist (PT) in the state of NJ. Previous home healthcare experience welcomed, but not required. Ability to travel to cases as assigned A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.
