Responsibilities Perform comprehensive PT evaluations and develop individualized treatment plans for rehab patients. Deliver evidence-based therapeutic interventions to restore mobility, strength, and function. Document care accurately in EMR and maintain regulatory and payer compliance. Collaborate with physicians, nurses, and allied health teams to coordinate care plans. Educate patients and families on home programs, safety, and recovery expectations. Track clinical outcomes and adjust treatment plans to optimize results. Uphold quality, safety, and professional practice standards in all patient care activities.
