Job Description

Jackson Therapy Partners is seeking a Physical Therapist to provide high-quality, evidence-based care across diverse clinical settings. In this role, you'll evaluate and treat patients to restore function, reduce pain, and improve mobility and quality of life. You'll collaborate with an interdisciplinary team, develop and implement individualized treatment plans, document progress, and educate patients and families. Responsibilities Perform comprehensive physical therapy evaluations, including history, systems review, and standardized tests and measures to determine functional deficits and impairments Develop individualized, evidence-based plans of care with clear, measurable goals in collaboration with patients, families, and the interdisciplinary team Provide skilled interventions such as therapeutic exercise, manual therapy, neuromuscular re-education, gait and balance training, and functional mobility training Educate patients and caregivers on diagnosis, treatment plans, home exercise programs, safe mobility, and use of assistive devices to promote independence and prevent re-injury Document evaluations, daily treatment notes, progress reports, and discharge summaries accurately and timely in accordance with regulatory, facility, and payer requirements Regularly reassess patient progress, modify treatment plans as clinically indicated, and communicate changes to the healthcare team and referring providers Collaborate with physicians, nurses, occupational and speech therapists, case managers, and other team members to coordinate care and participate in discharge planning Adhere to professional, ethical, and legal standards of practice, including maintaining patient confidentiality and complying with state practice acts and facility policies Adapt quickly to different clinical settings, patient populations, and EMR systems based on assignment requirements Participate in ongoing professional development, in-service training, and quality improvement initiatives supported by Jackson Therapy Partners Required Skills Physical therapy evaluation and differential diagnosis Development of individualized plans of care Therapeutic exercise prescription and progression Manual therapy techniques Neuromuscular re-education Gait and balance assessment and training Functional mobility and transfer training Orthopedic and post-surgical rehabilitation Neurologic rehabilitation (e.g., stroke, TBI, spinal cord injury) Geriatric rehabilitation and fall prevention Cardiopulmonary endurance and mobility training Use and fitting of assistive devices and durable medical equipment Patient and caregiver education Clinical documentation in EMR systems Outcome measure selection and interpretation Interdisciplinary team collaboration and communication Time management and caseload prioritization Knowledge of insurance, reimbursement, and regulatory requirements in rehab settings
