Job Description

Overview Join our dynamic healthcare team as a dedicated Physical Therapist, where you will play a vital role in restoring and enhancing patients' mobility, strength, and overall well-being. This position offers an exciting opportunity to work across diverse settings, including our outpatient clinic, hospital floor, and nursing home environment. Your expertise will directly impact patient recovery journeys by developing personalized treatment plans and collaborating with multidisciplinary teams to deliver exceptional care. We are committed to fostering a supportive environment that values your skills and promotes professional growth. Physical Therapist will work alongside a PTA and 2 PT Techs every shift. Duties Assess patients' physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to identify treatment needs. Develop and implement individualized care plans targeting rehabilitation goals for diverse populations such as geriatrics, pediatrics, neurology, orthopedics, and post-surgical patients. Deliver evidence-based physiotherapy interventions, including trigger point therapy and patient rehabilitation techniques, to improve mobility and reduce pain. Coordinate discharge planning and collaborate with healthcare professionals to ensure seamless patient transitions across care settings. Maintain accurate clinical documentation standards within electronic health records (EHR) systems, including detailed treatment notes and progress reports. Engage in patient education by communicating treatment goals clearly and providing guidance for home & community care routines. Conduct ongoing assessments to monitor progress and adjust treatment plans accordingly, ensuring optimal outcomes. Experience Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, inpatient units, or home health environments. Demonstrated expertise in outpatient rehabilitation center settings with familiarity in outpatient experience and outpatient clinic operations. Strong background working with geriatric patients and knowledge of geriatrics-specific care strategies. Hands-on experience with acute care settings, inpatient hospital environments, or outpatient rehabilitation centers is highly valued. Familiarity with medical terminology, clinical documentation standards, EMR (Electronic Medical Records) systems, and healthcare collaboration tools. (Athena and WebPT) Experience working with pediatric populations or neurological conditions is a plus. Ability to communicate effectively with clients and healthcare teams while adhering to physiotherapy best practices and safety protocols. Pay:
$124,000.00 - $155,000.00 per year
