Job Description

The Emerald Peek Rehabilitation and Nursing Center seeks a compassionate Physical Therapist to provide high-quality, resident-centered care in our skilled nursing and short-term rehabilitation facility in Peekskill, NY. In this on-site role, you will evaluate residents, develop and implement individualized treatment plans, and deliver therapeutic interventions that promote recovery, mobility, and functional independence. You will collaborate closely with an interdisciplinary team, contribute to care planning and discharge planning, and educate residents and families to support long-term wellness in a mission-driven, respectful, and collaborative environment. Responsibilities Perform comprehensive physical therapy evaluations and develop individualized treatment plans. Deliver evidence-based therapeutic interventions to improve mobility, strength, balance, and functional independence. Document assessments, treatments, and progress in EMR in accordance with regulatory and facility standards. Collaborate with nursing, physicians, and interdisciplinary team members on care plans and discharge planning. Educate residents and families on exercises, safety, and strategies to maintain functional gains. Participate in care conferences and quality improvement initiatives to support clinical excellence. Monitor patient progress, adjust treatment plans, and communicate changes to the care team. Ensure compliance with all state, federal, and facility policies and professional standards.
