Position Overview Elderwood at Cheektowaga is seeking a compassionate and dedicated Physical Therapist to join our skilled nursing facility team. Our Physical Therapy professionals play a vital role in ensuring the health, safety, and well-being of our residents by providing high-quality, individualized therapy services that support recovery, mobility, and independence. Responsibilities Provide comprehensive physical therapy evaluations and treatments for residents in a skilled nursing setting Develop and implement individualized treatment plans based on resident needs and physician recommendations Administer skilled physical therapy interventions to improve mobility, strength, balance, and overall function Monitor resident progress and adjust treatment plans as appropriate Collaborate with interdisciplinary team members to ensure coordinated and effective care Maintain accurate and timely clinical documentation in accordance with regulatory standards Ensure the highest degree of quality care and resident safety is maintained at all times Qualifications Current State Physical Therapy License required Strong clinical assessment and treatment planning skills Ability to work collaboratively in a multidisciplinary team environment Commitment to providing compassionate, resident-centered care Why Join Elderwood at Cheektowaga? Responsibilities:
Physical Therapist:
Initiates skilled physical therapy treatments in accordance with physician's orders; maintains required contact with physician. Maintains written evaluations, progress notes and discharge summaries for individuals in physical therapy program. Confers with other health care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function. Provides input for meetings and attends during absences of the Director Physical Therapy. Maintains statistical records of individuals' treatments. Assists with in-service training programs upon request. Records and updates individual's plan of care in the Inter-disciplinary Kardex. Submits requisition for supplies needed in the treatment to Director Physical Therapy. Assumes all duties of the Director Physical Therapy during absences including supervision of Physical Therapy Aides and responsibility for all necessary documentation.
Qualifications:
Physical Therapist :
Graduation from an approved school of physical therapy required. State license to practice physical therapy required. Two years' experience in physical therapy preferred. Ability to direct others necessary. This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
