Physical Therapist - Adult Homecare

- As we continue to expand our clinical programs to meet increasing community needs, we are seeking to fill positions in Onondaga, Cayuga, and Jefferson Counties.
Your responsibilities include:
Deliver safe, patient-centered physical therapy interventions in the home Perform comprehensive patient assessments using best clinical practices Develop, implement, and update individualized plans of care Serve as a key member of the interdisciplinary care team, communicating patient progress and care needs Educate patients and caregivers on mobility, safety, and therapeutic exercises Ensure accurate, timely, and clear documentation (including OASIS, within regulatory timeframes) Supervise and provide oversight of paraprofessional staff, including home health aides, as related to the patient's plan of care Maintain compliance with infection control, quality improvement, and regulatory standards Manage a diverse adult caseload while meeting productivity expectations Stay current with clinical best practices and participate in ongoing education and in-services What We're Looking For! Current New York State Physical Therapy license Registered with New York State and the American Physical Therapy Association (as applicable) Valid NYS driver's license and automobile insurance One year of physical therapy experience preferred (home care experience a plus) Strong clinical judgment, organization, and ability to work independently Excellent communication and documentation skillsPhysical Requirements:
Ability to sit or stand for long periods during work day Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 50 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs. Ability to drive in ALL weather conditions (rain, sleet, snow, wind). Visual/hearing ability sufficient to comprehend written or verbal communication Ability to use telephone and communicate over the phone Frequently uses a laptop computer to enter orders and obtain product information Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.Compensation & Benefits:
