Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A Clinical Pharmacist with infusion expertise is needed for a direct hire opportunity in Camillus, NY. This role focuses on preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications in a home infusion setting. Serving as the central figure in a multidisciplinary team, you will oversee sterile compounding, adjust medication doses, and provide essential education to patients and caregivers.
Core Responsibilities:
Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment adhering to state and federal guidelines. Assess laboratory values, calculate pharmacokinetic dosing (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to optimize patient treatments. Provide thorough counseling and education regarding specialty medications and treatment plans. Teach patients and caregivers proper use of infusion pumps, sterile technique, and catheter maintenance at home. Collaborate with prescribing physicians, home health nurses, and internal teams (intake, referral, reimbursement, clinical services, pharmacy operations) to ensure coordinated care and prevent hospital readmissions. Maintain regulatory compliance with State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other standards. Manage pharmacy inventory including refrigerated items, and prepare medications and supplies for timely and safe delivery.
Qualifications:
Minimum of 2 years' experience in infusion specialty pharmacy. Experience with IV immunoglobulin (IVIg) is advantageous. Familiarity with specialty therapy payer requirements, reimbursement, and contract obligations. Current and active NY pharmacist license in good standing.
