5 days/week Pride Health is hiring a Home Health Physical Therapist (PT) to support our client's patients in Kingston, NY, with coverage throughout Kingston and the southern Ulster County region.

Pay Range:
$70 to $75/ hr on W2 Based on experience and qualifications
Mileage Reimbursement Rate:
Reimbursement provided from first patient to last patient
Weekend Requirement:
1 weekend per month
On-Call:
None Duties:
Perform comprehensive OASIS assessments, including SOC, recertification, reassessment, and discharge evaluations using HCHB. Conduct PT evaluations and develop individualized plans of care based on strength, mobility, balance, gait, pain, and functional needs. Provide skilled PT treatments, reassessments, and follow-up visits in the home setting. Educate patients and caregivers on exercises, fall prevention, safety, mobility, and disease management. Collaborate with physicians, PTAs, caregivers, and interdisciplinary teams while maintaining accurate HCHB documentation.
Requirements:
Education:
Graduate of an accredited Physical Therapy program (Master's or higher) License(s): Active, unrestricted Physical Therapist (PT) license Certification(s): As required by facility/state Years of experience: Minimum 1 year of clinical experience; Home Health experience required
