The Physical Therapist provides Physical Therapy (P.T.) services, ensuring high-quality care in compliance with federal, state, and local regulations, aligned with the mission of the Jewish Home of Rochester. Supervises P.T. Assistants, volunteers, Physical Therapy students and per diem Physical Therapists. Conduct patient evaluations and create care plans per physician orders. Provide guidance to nursing staff on MDS and GG assessments. Serve on committees and conduct training programs for staff. Develops an aftercare exercise program when indicated and instructs other patient care personnel in proper procedures. Act as a clinical instructor for P.T. students. Support in-service programs and facility initiatives. What do I need for this role? Degree in Physical Therapy from an accredited institution. Valid New York State Physical Therapy license. Basic knowledge relative to the practice of physical therapy. Basic familiarity with equipment used in the practice of physical therapy and its operation. One (1) year experience in a therapist capacity in a hospital, skilled nursing care facility or other related medical facility preferred.
