Our Skilled Nursing Facility in New City located in Rockland County, NY is looking for a Per diem Physical Therapist to join our team.

Key Responsibilities:

Evaluate Residents condition and develop an appropriate physical rehabilitation plan. Communicate with residents, families, referring physicians and other members of the team. Provide timely written documentation per facility and department requirements. Develop and maintain physical therapy standards. Meet with administration, medical and nursing staff, as well as other related departments in planning therapy services. Provide timely written documentation as required by insurance provider to meet licensing, certification regulatory, reimbursement requirements and physician treatment plan. Support and participate in department operations and development.

Requirements:

Must be dynamic, team-oriented and enjoys working with a variety of patients. Must have a valid Physical Therapist License in the state of NY. Must have at least 2 years' experience as a PT in a
