Job Description
Sunshine Homecare is looking to hire dedicated an experienced Per-Diem Physical Therapist to join our clinical team in the provision of comprehensive and quality rehabilitative care to patients at home with the goal of helping them regain mobility, strength and function. Opportunities available in Rockland, Westchester, Orange and Dutchess Counties.
Conduct comprehensive assessments and develop treatment plans towards achieving desired goals.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Communicate with nursing and medical team regarding patient's progress towards goals.
Instructs family members, Home Health Aides, and other caregivers in appropriate physical therapy techniques and what to expect from the recovery process.
Complete required documentation timely in agency EMR in accordance with agency and regulatory requirements.
Physical Therapist Responsibilities:
Travel to and from patients' homes, providing physical therapy as ordered by a physician in accordance with the designated plan of care.Conduct comprehensive assessments and develop treatment plans towards achieving desired goals.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Communicate with nursing and medical team regarding patient's progress towards goals.
