Job Description

The Emerald Peek Rehabilitation and Nursing Center seeks a compassionate Physical Therapist to provide resident-centered care in our skilled nursing and short-term rehab facility in Peekskill, NY. In this role, you will perform comprehensive evaluations, develop and implement individualized treatment plans, and deliver evidence-based interventions to support functional recovery, mobility, and safety. You will collaborate closely with an interdisciplinary team, educate residents and families, document progress, and contribute to a dignified, respectful, and clinically excellent care environment focused on quality of life and meaningful outcomes. Responsibilities Perform comprehensive PT evaluations and develop individualized treatment plans for subacute and long-term care residents. Deliver evidence-based therapeutic interventions including exercise, gait training, and balance activities. Document assessments, progress notes, and discharge summaries accurately in the EMR. Collaborate with nursing, rehab, and medical teams to coordinate resident-centered care plans. Educate residents and families on mobility, home exercise programs, and safety strategies. Participate in care conferences and contribute to discharge planning and transition of care. Monitor resident progress and adjust treatment goals and interventions as needed. Ensure compliance with state, federal, and facility regulations and PT best practices.
