Responsibilities:
Accept and treat patients within your designated geographic area Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plansDevelop a plan of care and treatment goals in consultation with the physician, patient, and familyPlan, prepare and implement customized therapy programs to restore function and promote independence Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniquesProvide timely and accurate documentation for each visitCommunicate effectively with patients, families, and interdisciplinary team members, especially around discharge planningDeliver care in compliance with agency policies and professional practice standardsQualificationsPhysical Therapist Program GraduateLicensed Physical Therapist in the state of New York or eligible.

Home health experience is preferred but not required•We will provide you with a mentor to help with documentation and compliance.

Ability to travel to patients' homes within the assigned geographic territoryAdditional InformationThe anticipated pay range for this position is $75-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
