As a School Based Physical Therapist, you'll play a crucial role in helping us achieve our mission. About the Role We're seeking a passionate and experienced School Based Physical Therapist to join our team in Patchogue, NY . As a key member of our team, you'll provide physical therapy services to students in a school setting, helping them to overcome physical challenges and achieve their full potential. Responsibilities Provide physical therapy services to students in a school setting, including assessing, planning, and implementing individualized treatment plans Collaborate with teachers, families, and other healthcare professionals to develop and execute comprehensive care plans Conduct evaluations, assessments, and goal setting with students, families, and school staff Develop and implement creative and engaging physical therapy programs to meet the unique needs of each student Maintain accurate and detailed records of treatments, progress, and outcomes Participate in IEP meetings and other multidisciplinary team meetings as needed Requirements Master's degree in Physical Therapy from an accredited program New York State license to practice as a Physical Therapist Knowledge of HIPAA regulations and ability to maintain strict confidentiality of student and client information.
