Job Description

The Emerald Peek Rehabilitation and Nursing Center seeks a compassionate Physical Therapist to provide resident-centered care in our skilled nursing and short-term rehab facility in Peekskill, NY. The Physical Therapist will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility, function, and safety. Working within an interdisciplinary team, you will collaborate with nursing, physicians, and rehab staff, educate patients and families, and document progress to support recovery, wellness, and quality of life in a mission-driven environment. Responsibilities Perform comprehensive physical therapy evaluations for subacute and long-term care residents Develop and implement individualized treatment plans focused on mobility, function, and safety Deliver therapeutic exercises, gait training, balance and transfer training Collaborate with nursing, rehab, and medical teams on resident care plans and goals Educate residents and families on home exercise programs, equipment, and safety Document assessments, goals, progress notes, and discharge summaries in EMRParticipate in care conferences and quality improvement initiatives Ensure compliance with regulatory, ethical, and facility standards of care Required Skills Physical therapy evaluation Treatment planning Gait training Therapeutic exercise Neuromuscular re-education Wound care coordination Care documentation (EMR) Interdisciplinary care planning Patient and family education Discharge planning Falls prevention Pain management
