Job Description

Sigma Rehab is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will assess, plan, and implement individualized treatment programs to help patients restore function, reduce pain, and improve mobility. You will collaborate closely with an interdisciplinary team, educate patients and families, and document progress to ensure optimal outcomes. Responsibilities Perform comprehensive physical therapy evaluations, including musculoskeletal, neurological, and functional assessments for a diverse patient population. Develop and implement individualized, evidence-based treatment plans focused on restoring function, improving mobility, and reducing pain. Deliver hands-on interventions such as manual therapy, therapeutic exercise, neuromuscular re-education, balance and gait training, and use of appropriate modalities. Educate patients and families on diagnosis, treatment goals, home exercise programs, injury prevention, and strategies to maximize independence and quality of life. Collaborate with an interdisciplinary team, including physicians, nurses, and other rehab professionals, to coordinate and optimize patient care plans. Document evaluations, daily treatment notes, progress reports, and discharge summaries accurately and in a timely manner within the EMR system. Monitor and adjust treatment plans based on patient progress, clinical judgment, and outcome measures to ensure safe and effective care. Maintain compliance with all professional, legal, and regulatory standards, including infection control and safety protocols. Participate in staff meetings, case conferences, and in-service training to support continuous quality improvement and professional development. Contribute to a supportive, patient-centered culture by demonstrating empathy, clear communication, and respect for patients, families, and colleagues.
