Duties Conduct comprehensive patient evaluations to determine physical therapy needs across various settings such as outpatient clinics, hospitals, or home health environments. Develop and implement tailored treatment plans utilizing techniques like trigger point therapy, orthopedics, neurology interventions, and geriatrics-focused therapies. Manage discharge planning processes to ensure seamless transition of care from hospital to home or community settings. Document all clinical interactions accurately in electronic health records (EHR) systems following clinical documentation standards. Collaborate effectively with healthcare professionals including physicians, nurses, and occupational therapists to optimize patient outcomes. Educate patients and their families on treatment exercises, injury prevention strategies, and overall physiotherapy practices to promote ongoing wellness. Participate in outpatient rehabilitation center activities, inpatient care routines, and home & community care programs to support diverse patient needs. Requirements Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home health settings. Strong knowledge of physiology, anatomy, medical terminology, and treatment modalities such as trigger point therapy. Experience with outpatient rehabilitation centers or inpatient hospital environments is highly preferred. Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for accurate documentation. Ability to develop effective treatment plans based on comprehensive assessments and patient goals.
