Job Description
All the duties outlined within this document will be performed to established policies, procedures and guidelines within the department and HHS. Works under the supervision of the Manager and Director of Therapy Services as well as Medical Staff. Performs direct patient care within the guidelines of the referring to evaluate, design and reassess age-specific treatment plans to meet the assessed needs of pediatric to geriatric patients; and performs adequate documentation. Supervises Physical Therapy Assistants and unlicensed support personnel in therapy department. Assists in formulation of departmental and hospital-wide PI activities. Performs other duties as assigned.
Education, Work Experience and Licensure:
- Bachelor's/Master's or Doctoral Degree or certificate from a Physical Therapy Program this is accredited by the American Physical Therapy Association. (Graduated from a CAPTE approved program in PT for graduates on/after December 31, 2009).
- Current Ohio Physical Therapy license in good standing.
- May be entry, but one-year experience preferred.
