Advance your career as a Home Health Physical Therapist by providing vital rehabilitation and therapeutic care to patients in their homes. This contract position involves helping individuals improve their mobility, strength, balance, and independence while adhering to high clinical standards. Youll play a key role in collaborating with a multidisciplinary team and ensuring tailored, patient-centered support throughout the care process.

Schedule/Hours:

Full-time, 13-week contract Monday through Friday, 8:00 AM 5:00 PM Field-based with daily travel to patient homes (mileage reimbursement provided) Typical caseload: 2532 points weekly (approximately 57 patient visits per day, depending on visit types)

Key Responsibilities:

Complete comprehensive physical therapy evaluations, including OASIS assessments as required Develop and implement individualized plans of care prioritizing functional mobility, strength, balance, gait, pain management, and safety Deliver interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention strategies Monitor patient progress, adjust care plans, and communicate changes to physicians and colleagues Educate patients and caregivers on exercises, safety, and assistive devices Accurately document all care in Home Care Home Base (HCHB) Participate in interdisciplinary meetings, case conferences, and ongoing education Comply with regulatory standards, Medicare guidelines, and agency policies

Qualifications:

Active and unrestricted Physical Therapist (PT) license for the state At least 1 year of recent home health physical therapy experience (field experience preferred) Proficiency with Home Care Home Base (HCHB) and OASIS required Excellent clinical decision-making and assessment skills Reliable transportation, valid drivers license, and insurance Ability to work independently, organize a flexible field schedule, and communicate effectively
