With over 30 years of experience, we are committed to promoting health and healing within our community. Overview We are seeking a dedicated and skilled Physical Therapist to join our dynamic team. The ideal candidate will possess a strong background in physiotherapy practices, with experience working with diverse patient populations. This role involves developing personalized treatment plans, collaborating with healthcare professionals, and delivering exceptional patient care across outpatient, inpatient, and home health settings. Duties Assess patients' physical conditions through detailed evaluations and develop tailored treatment plans to promote recovery and improve mobility. Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to facilitate patient rehabilitation. Monitor patient progress regularly and adjust treatment strategies accordingly to ensure optimal outcomes. Collaborate effectively with physicians, nurses, and other healthcare providers to coordinate care and discharge planning. Maintain accurate clinical documentation in accordance with medical terminology standards and electronic health records (EHR) management protocols. Educate patients and their families on treatment procedures, home exercises, and community resources to support ongoing recovery. Ensure compliance with clinical documentation standards, safety protocols, and healthcare regulations across all practice settings. Requirements Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or inpatient settings. Strong knowledge of physiology, anatomy, orthopedics, neurology, and geriatrics to address diverse patient needs. Experience working with geriatric patients and pediatric populations in outpatient or home health environments. Proficiency in outpatient rehabilitation center practices including discharge planning and care plan development. Familiarity with electronic health records (EHR) systems such as EMR platforms for accurate documentation. Excellent client communication skills coupled with the ability to collaborate effectively within multidisciplinary teams. Knowledge of medical terminology relevant to physiotherapy treatments and procedures. Prior experience in outpatient clinic or hospital environments emphasizing patient care and rehabilitation services is highly desirable.
