Job Description

Wapak Manor is under new management and building a strong, dedicated team. We are looking for a Physical Therapist (PT) who wants to be part of a facility that is focused on improving care, teamwork, and outcomes. Responsibilities:

Evaluate and treat residents to improve mobility and strength Develop and implement individualized care plans Work closely with nursing and interdisciplinary team Maintain accurate and timely documentation Educate residents and families on therapy plans

Qualifications:

Licensed Physical Therapist in the state of Ohio Long-term care experience preferred (not required) Strong communication and organizational skills
