We are seeking a compassionate and dedicated PRN Physical Therapist to join our rehabilitation team and support residents in achieving their highest level of function and independence. Position Summary The PRN Physical Therapist provides highquality rehabilitative services to residents in our skilled nursing and assisted living settings. This role focuses on evaluating, planning, and implementing individualized treatment programs to improve mobility, strength, safety, and overall quality of life. Key Responsibilities Conduct comprehensive physical therapy evaluations for new and existing residents Develop and implement individualized treatment plans based on clinical findings Provide therapeutic interventions including strengthening, mobility training, balance work, and functional activities Document evaluations, progress notes, and discharge summaries in accordance with facility and regulatory standards Collaborate with nursing, therapy, and interdisciplinary team members to support resident goals Educate residents, families, and staff on safe mobility techniques and home exercise programs Participate in care plan meetings as needed Ensure compliance with all state, federal, and facility guidelines Qualifications Current Ohio Physical Therapist (PT) license in good standing Graduate of an accredited Physical Therapy program Experience in skilled nursing or longterm care preferred Strong clinical reasoning, communication, and documentation skills Ability to work independently and adapt to changing resident needs Compassionate, patientcentered approach to care Schedule PRN / AsNeeded Flexible scheduling based on facility needs
