• $150 Eval (complex skill add-on)
• $115 Revisit / Discharge
• $70 Downtime (pre-approved wait time)
• $45/hr Premium add-ons: Same-Day /

Holiday:

Requirements Must-Have:

Active PT license (Ohio) + current BLS certification Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area Minimum 1 year of clinical physical therapy experience Preferred /

Nice-to-Have:

OASIS certification or demonstrated OASIS experience Prior home health experience Familiarity with HCHB or similar home health
