The Role - Physical Therapist (PT) The Physical Therapist will provide skilled physical therapy services primarily in patients' private homes throughout Lorain County, Ohio . The PT will evaluate patients, develop individualized plans of care, provide skilled therapeutic interventions, monitor progress, and collaborate with patients, families, physicians, and other members of the care team. This position offers a high degree of clinical and scheduling autonomy , while still providing the support and structure needed to be successful. In this role, you will: Perform comprehensive physical therapy evaluations Develop individualized plans of care Provide skilled physical therapy interventions in patients' homes Monitor patient progress and modify treatment plans as appropriate Establish strong relationships with patients and families Communicate with physicians and other members of the care team Manage your own daily schedule and visit flow Complete timely and accurate clinical documentation Maintain a high level of clinical and professional accountability Deliver a premium, relationship-centered patient experience The territory is designed to be geographically concentrated , helping minimize unnecessary drive time and maximize productive clinical time. Territory - Lorain County, Ohio This position primarily covers Lorain County and surrounding communities . The organization intentionally maintains manageable territories so clinicians can build an efficient caseload without spending excessive amounts of time traveling between distant visits.

More time with patients. Less time behind the wheel. Compensation - Point-Based Pay The organization utilizes a transparent point-based compensation model rather than traditional hourly compensation. Each visit is assigned a point value based on factors such as: Time Complexity Clinical responsibility Type of service provided Point compensation ranges from $57-$66 per point, based on experience and qualifications. This structure allows compensation to directly reflect the clinician's level of experience while providing meaningful earning potential as productivity increases. There is no cap on earning potential , giving clinicians meaningful control over their workload and income. Performance Bonus Once a clinician exceeds 30 points per week , additional points are eligible for a 10% bonus rate . This provides an additional opportunity for clinicians who want to increase their weekly visit volume and earnings. How the Point System Works Instead of being paid strictly by the hour: Each visit carries a designated point value More complex or involved visits carry greater point values Points directly determine compensation The per-point rate is determined based on experience and qualifications Higher productivity can translate into higher earnings The model is designed to provide clarity, flexibility, and direct alignment between productivity and compensation.
