Job Summary Join our dynamic healthcare team as a full-time Outpatient Physical Therapist and make a meaningful difference in patients' lives! Your expertise in physiotherapy will help patients regain mobility, reduce pain, and improve their overall well-being through tailored interventions rooted in solid anatomy and physiology knowledge. Responsibilities Assess patients' physical conditions and establish comprehensive care plans aligned with clinical standards and patient goals. Implement evidence-based treatment strategies such as manual therapy, trigger point therapy, and therapeutic exercises to facilitate patient rehabilitation. Document all patient interactions accurately using electronic health records (EHR) systems while adhering to clinical documentation standards. Coordinate discharge planning and collaborate with multidisciplinary teams—including physicians, nurses, and occupational therapists—to ensure seamless patient care transitions. Manage patient progress through ongoing evaluations and modify treatment plans as needed to optimize outcomes. Work with diverse populations including geriatric patients, pediatrics, neurology cases, orthopedics, and those requiring home health or outpatient rehabilitation services. Educate patients and their families on treatment procedures, self-care techniques, and community resources to support recovery at home or in the community. Qualifications Valid licensure as a licensed Physical Therapist in the relevant state or jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospital settings, or home health environments. Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards. Experience working with geriatric patients, pediatrics, neurology cases, orthopedics or outpatient rehabilitation centers is highly preferred. Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and treatment tracking. Ability to develop effective treatment plans based on comprehensive assessments and collaborate effectively with healthcare teams.
