You're compassionate, collaborative, and skilled in guiding individuals through customized rehabilitation plans. Responsibilities Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position: Performs and documents an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention. Identifies and documents goals, anticipated progress, and plans for reevaluation. Plans, prepares, or carries out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients. Records prognosis, treatment, response, and progress in patient's chart or enters information into computer. Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit. Confers with the patient, medical practitioners, or other appropriate parties to plan, implement, or assess the intervention program. Complies with professional, ethical, and legal standards.Required Education and Experience Education:
Graduate of an accredited physical therapy programExperience:
At least 2 years' experience as a physical therapistLicenses/Certifications:
