This position is scheduled to work 40 hours\/week, day shift, primarily home health, Monday-Friday, and includes weekend on-call rotation in acute care and home health.Rehab Department Hours :
Hours of operation are Monday-Thursday, 7:00am-6:30pm, and Friday, 7:00am-5:00pm. Saturday and Sunday are on an on-call, as needed basis. The Rehab Department utilizes Epic.Primary Responsibilities:
Patient evaluations- Perform comprehensive assessments and review medical histories and physician referrals to determine therapy needs Establish goals to enhance independence with daily living
- Develop personalized care plans based on patient goals, diagnosis, and progress and adjust as needed Therapeutic Interventions
- Administer therapeutic exercises, manual therapy, and modalities such as ultrasound or electrical stimulation, gait training, posture correction, and pain management strategies Communicate and counsel patients and families on lifestyle modifications and injury prevention while coordinating care with physicians, occupational therapists, and other healthcare professionals You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
Required Qualifications:
Graduate of an accredited College or School of Physical Therapy Bachelor's degree Current Ohio Physical Therapist license in good standing Driver's License and access to reliable transportationPreferred Qualifications:
Masters' degree Experience in a hospital setting 1+ years of work experience in home health, and willing to train. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $72,800 to $130,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
