Job Description

Otterbein Senior Life seeks a compassionate Physical Therapist to provide resident-centered rehabilitation services to older adults in our faith-based senior living community. In this role, you will assess mobility, strength, and functional abilities; create individualized treatment plans; and deliver therapeutic interventions to improve independence, safety, and quality of life. You will collaborate with an interdisciplinary team, educate residents and families, and maintain accurate documentation while contributing to a supportive, mission-driven environment focused on dignity and respect. Responsibilities Evaluate residents' functional mobility, strength, balance, and pain levels Develop and implement individualized treatment plans for older adults Provide therapeutic exercises, manual therapy, and modalities to improve function Collaborate with interdisciplinary care teams to support resident-centered goals Educate residents, families, and staff on home exercise and safety programs Document assessments, progress notes, and care plans in accordance with regulations Support discharge planning and rehabilitation goals for post-acute patients Participate in quality improvement and evidence-based practice initiatives Required Skills Physical therapy evaluation and treatment planning Geriatric rehabilitation Balance and fall prevention Therapeutic exercise prescription Manual therapy techniques Patient and family education Electronic medical records documentation Interdisciplinary care coordination Pain management strategies Knowledge of Medicare and insurance regulations
