Shift:
- Part Time and PRN
- Responsibilities
- Provide physical therapy services to patients according to a plan of care. This may include assessing and evaluating therapeutic/rehabilitative/functional status and participation in developing the total plan of care as well as providing direct physical therapy treatment.
- Assess patients for muscle strength, mobility, gait, ROM and transfer capabilities.
- Instruct patients, families and caregivers in the use and care of therapeutic appliances.
- Report patient's reaction to treatment or changes in care to patient's physician as needed and appropriate.
- Initiate physical therapy programs and instruct other personnel, patient and/or family/caregivers in their therapy program, as well as the goals of therapy.
- Supervise the Physical Therapist Assistant (PTA) as necessary.
- Participate in Otterbein Universal Home Health Quality Assessment/Performance Improvement program.
- Confer with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care.
- Prepare and submit clinical and progress notes in a timely manner and in accordance with Otterbein Universal Home Health, state and federal guidelines and regulations.
- Education and Experience
Education:
Bachelor's degree or master's in physical therapy.
Experience:
Minimum 2 years of physical therapist experience with community / home health preferred.- Licensure, Registration, and/or
Certification:
Current license as a Physical Therapist from the State of Ohio or made application for same. Must have a valid Ohio driver's license and access to reliable transportation. Current CPR certification required.- Complete a minimum of twelve (12) hours of in-services per year.
