Pride Health is hiring a Physical Therapist - Home Health to support our client's healthcare facility in Wilmington, OH.

Pay Range:

$50.00 to $57.00/hour

Mileage Reimbursement Rate (if applicable): $0.51/mile

•Pay offered is based on experience, expertise, credentialing, and education.

Duties:

Provide skilled home health PT services, including gait/balance training, strengthening, mobility improvement, and home safety interventions based on individualized plans of care.

Maintain accurate clinical documentation, coordinate with the care team, monitor patient progress, and meet the 30-point weekly productivity expectation.

Requirements:
  • Current and active Physical Therapist (PT) license in the State of Ohio.
  • Minimum 1 year of Home Health Physical Therapy experience.
  • Strong clinical assessment, treatment planning, and patient education skills.
  • Ability to work independently and manage a patient caseload effectively.
  • Excellent communication and interpersonal skills.
