The ideal candidate will possess a strong background in physiotherapy, with expertise in developing treatment plans, managing patient care, and collaborating with healthcare professionals. This role offers an opportunity to make a meaningful difference by providing high-quality outpatient and home health rehabilitation services to diverse patient populations, including geriatric, pediatric, neurological, and orthopedic cases. Duties Assess patients' physical conditions and develop individualized treatment plans aimed at improving mobility, strength, and functional independence. Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs tailored to each patient's needs. Monitor patient progress regularly and adjust treatment strategies accordingly to optimize outcomes. Collaborate with physicians, nurses, and other healthcare professionals to coordinate care plans and discharge planning processes. Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards. Educate patients and their families on exercises, mobility techniques, and home care strategies to promote ongoing recovery outside clinical settings. Manage discharge planning effectively by ensuring patients are equipped with appropriate care instructions for home or community-based care environments. Maintain compliance with healthcare regulations and safety standards across inpatient, outpatient, hospital, and home health settings. Requirements Valid licensure as a licensed Physical Therapist in the relevant jurisdiction with practicing experience in physiotherapy roles. Demonstrated experience working in outpatient clinics, inpatient hospital settings, or home health environments. Proficiency in clinical documentation standards and electronic health records (EHR/EMR) management systems. Ability to collaborate effectively with multidisciplinary healthcare teams and communicate clearly with patients of diverse backgrounds.
