Overview Physical Therapist - Outpatient, travel assignment in Lebanon, Oregon, starting 09/28/2026 for 13 weeks.
Weekly pay range:
$2,365-$2,423 per week.
Guaranteed hours:
40 hours per week.
Shift:
Days, 4x10-hour shifts (0700-1700) with no weekend coverage.
Work setting:
hospital-based outpatient rehabilitation department.

Setting:
Hospital-based outpatient rehabilitation department Responsibilities Conduct initial evaluations and ongoing treatment for outpatient patients with injuries or conditions affecting mobility and functional independence. Develop, implement, and adjust individualized rehabilitation plans; monitor progress and modify interventions as needed. Provide therapy interventions such as therapeutic exercise, gait training, manual therapy, and modalities within scope and department guidelines. Document patient assessments, treatment plans, goals, and progress in the medical record; communicate with the care team and document patient education provided. Educate patients and families on home exercise programs, safety strategies, and functional adaptations to support continued recovery. Collaborate with a multidisciplinary team to coordinate care and ensure seamless patient transitions across services.
