Our team is committed to delivering compassionate, high-quality care to individuals in the comfort of their own homes. We're currently seeking a full-time or part-time, licensed Physical Therapist to join our dynamic team. Immediate openings are available, coverage areas throughout our 60-mile service region in the beautiful Willamette Valley. Position Highlights Assigned territory based on patient need and your location preference See approximately 5 patients per day with a consistent caseload of ~25 patients Supportive, collaborative team environment Clinical Fellowship opportunities available Bonus opportunities — averaging $1,200 each month More than $750 each year toward continuing education Key Responsibilities Provide skilled and rehabilitative physical therapy to patients in their homes Perform manual and functional therapy exercises tailored to each patient Conduct initial evaluations and create customized Plans of Care Collaborate with caregivers and interdisciplinary team members Supervise PTAs and CNAs as needed Adjust treatment plans based on patient progress Complete accurate, timely Point-of-Care documentation Meet agency productivity expectations Qualifications Graduate of an accredited Physical Therapy program Active Oregon Physical Therapist license Valid Oregon driver's license and auto insurance CPR certification Reliable vehicle for travel Strong critical thinking and case management skills Ability to work effectively within a multidisciplinary team Previous home health experience preferred, but not required Why You'll Love Working Here Medical, Dental & Vision Insurance 401(k) with company match Paid Time Off + 6 Paid Holidays + 2 Bonus PTO Days Career development & training opportunities Continuing education support Fun extras like free frozen yogurt, espresso, and discounted gym membership
