40 hours/week Physical Therapist•Home Health | Northeast Philadelphia & Montgomery County, PA We're looking for a compassionate and motivated Physical Therapist to provide skilled home health services throughout Northeast Philadelphia and Montgomery County, PA. In this role, you'll help patients recover from illness, injury, or surgery while improving mobility, restoring function, and promoting independence—all in the comfort of their own homes. If you enjoy building meaningful patient relationships and working independently while supported by a collaborative healthcare team, we'd love to hear from you. Responsibilities Evaluate patients and develop individualized physical therapy treatment plans. Provide therapeutic exercises, gait training, balance training, transfer training, and functional mobility interventions. Assess strength, range of motion, balance, coordination, and functional limitations. Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques. Monitor patient progress and modify treatment plans based on clinical outcomes. Complete accurate and timely documentation in the electronic medical record (EMR). Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the healthcare team. Maintain compliance with home health regulations, documentation standards, and patient safety guidelines. Travel throughout the assigned service area to provide in-home patient care. Qualifications Current Physical Therapist (PT) license required. Home health experience is preferred but not required. Strong clinical assessment and treatment planning skills. Experience treating orthopedic, neurological, geriatric, and post-surgical patients preferred. Excellent communication, time management, and organizational skills. Ability to work independently and efficiently manage a daily caseload. Experience using electronic medical records (EMR) preferred.
