Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. Responsibilities Conduct comprehensive patient evaluations to determine physical therapy needs, including assessments of physiology, anatomy, and neurological function. Develop and implement tailored treatment plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and other modalities to promote rehabilitation. Collaborate effectively with healthcare professionals such as physicians, nurses, occupational therapists, and social workers to coordinate care and optimize patient outcomes. Manage discharge planning processes by preparing detailed care plans for patients transitioning between care settings or returning home. Maintain accurate clinical documentation in accordance with medical terminology standards and electronic health records (EHR) or electronic medical records (EMR) systems. Provide patient education on injury prevention, post-treatment exercises, and community resources for ongoing support. Engage with diverse patient populations including geriatric patients, pediatrics, neurology cases, orthopedics, and those requiring outpatient or home health services. Experience Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospital settings, or home health environments. Demonstrated knowledge of physiotherapy techniques including trigger point therapy and treatment plan development. Familiarity with clinical documentation standards and managing electronic health records (EHR) or electronic medical records (EMR) systems. Experience working with geriatric patients and understanding the unique needs of older adults in geriatrics or community care programs. Strong background in outpatient rehabilitation center operations, inpatient hospital care, or outpatient clinic environments. Ability to communicate effectively with clients of all ages while demonstrating empathy and professionalism in patient care delivery. Knowledge of medical terminology related to orthopedics, neurology, pediatrics, and general physiology to support accurate assessments and treatment strategies.
