Non-Exempt under the Fair Labor Standards Act (FLSA) and applicable Pennsylvania law OneWell Health Care is seeking a compassionate Physical Therapist (PT) to provide personalized therapy to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings in Pennsylvania. You will evaluate each individual's physical needs, develop goal-based treatment plans, and collaborate with caregivers and other professionals to improve mobility, strength, balance, safety, and overall independence. As a Physical Therapist, You'll…: Evaluate mobility, strength, balance, gait, posture, and safety needs. Develop and update individualized treatment plans. Provide evidence-based physical therapy in home and community-based settings. Help individuals improve mobility, coordination, physical function, and independence. Recommend mobility aids, adaptive equipment, therapeutic exercises, and fall-prevention strategies. Educate individuals, families, and caregivers on safe mobility techniques. Collaborate with caregivers and interdisciplinary team members. Complete accurate and timely clinical documentation. Participate in required meetings and trainings.
What We're Looking For:
: Doctorate or Master's degree in Physical Therapy from an accredited program. Current and valid Physical Therapist license in the state of practice. Current CPR certification (preferred or required in accordance with organizational policy). Strong clinical, documentation, organizational, and communication skills. Proficiency in functional mobility and gait assessments, balance and postural control interventions, adaptive and assistive equipment recommendations, evidence-based physical therapy techniques, and electronic health record (EHR) systems. Valid driver's license, a clean driving record, reliable transportation, and willingness to travel to home and community-based service locations. Flexibility to support varying schedules and assigned service areas is required. Experience supporting individuals with Intellectual and Developmental Disabilities (IDD) and providing physical therapy services in home or community-based settings is preferred. English proficiency is required. Bilingual proficiency (including Spanish) is a plus.
