Job Duties:
Evaluate residents and develop individualized treatment plans. Provide evidence-based physical therapy services to improve mobility, strength, balance, endurance, and functional independence. Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families to achieve the best outcomes. Educate residents and caregivers on treatment plans, exercises, and injury prevention. Accurately document evaluations, treatment sessions, progress notes, and discharge summaries. Participate in interdisciplinary care planning and quality improvement initiatives. Help residents safely return to their highest level of function and independence. Qualifications Current Physical Therapist license in the applicable state. Graduate of an accredited Physical Therapy program. Strong clinical, communication, and interpersonal skills. Passion for providing exceptional resident-centered care. Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required. Why WeCare Centers?
