Job Description
The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician's orders when applicable.
Essential Duties and Responsibilities:
- + Performs initial comprehensive assessment and reassessments and regularly reevaluates patient for rehabilitation needs, and works in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriate + Participates in developing the plan of care in collaboration with the physician and other members of the health care team + Assesses and evaluates and/or assists the physician in assessing and evaluating therapeutic, rehabilitative, function status, and home environment (as applicable) + Assesses muscle strength, mobility, gait, and Range of Motion (ROM) + On incisions resulting from orthopedic surgery, removes staples or sutures and manages incision line + Provides physical therapy treatment and establishes physical therapy plan of care + Instructs patient, families, and/or caregivers in the use of and care of therapeutic appliances + Determines priority needs for physical therapy + Reports patients' reaction(s) to treatment or changes in condition to physician and/or designee + Initiates physical therapy program and instructs, consults, and/or advises other personnel and/or family/caregiver regarding certain phases of physical therapy with which they may work with the patient, and explains the goals of the physical therapy program by participating in case conferences + Trains patient in use of prosthetic device(s) and use of other assistive devices (e.
Minimum Requirements:
