Physical Therapist (PT)
Pay:
$90.00-$95.00 per visit Home Health | Per Diem & Part-Time | Flexible Scheduling CarePine Home Health is hiring Physical Therapists (PTs) to provide one-on-one home health therapy in Bucks and Montgomery counties throughout : Souderton, Telford, Perkasie, Coopersburg, Sellersville, Harleysville, and surrounding areas. Responsibilities include:
Assess and evaluate patients' therapeutic, rehabilitative, and functional status Participate in the development and implementation of individualized plans of care Evaluate the patient's home environment and functional needs Assess muscle strength, mobility, gait, range of motion, and rehabilitation potential Develop and provide appropriate physical therapy treatment Educate patients and families/caregivers regarding treatment plans, therapeutic exercises, equipment, and assistive devices Monitor patient progress and response to treatment Report significant changes in patient condition to the physician and appropriate members of the care team Identify needs for additional home health services and make appropriate referrals Complete clinical documentation, progress summaries, and required reports accurately and on time Participate in discharge planning Coordinate equipment and other services necessary to support the patient's plan of care Provide supervision and evaluation of physical therapy aides when applicable Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team Qualifications Current and active Physical Therapist license in Pennsylvania Current CPR certification Strong verbal and written communication skills Strong organizational and time-management skills Ability to work independently in a home health environment Community or home health experience preferred About CarePine Home Health CarePine Home Health is a Medicare- and Medicaid-certified home health agency providing compassionate, patient-centered care in the communities we serve.
