Job Overview We are seeking a passionate and energetic Physical Therapist to join our dynamic team. In this role, you will develop and implement tailored treatment plans to help patients improve mobility, strength, and overall function. Responsibilities Conduct thorough assessments of patients' physical conditions to develop personalized treatment plans. Implement therapeutic interventions focused on patient rehabilitation, mobility enhancement, and pain management. Manage discharge planning and coordinate care with other healthcare providers to ensure seamless patient transitions. Document patient progress accurately using electronic health records (EHR) systems while maintaining compliance with clinical documentation standards. Provide education and guidance to patients and their families on exercises, safety measures, and recovery strategies. Collaborate with multidisciplinary teams including physicians, nurses, occupational therapists, and social workers to optimize treatment outcomes. Why Rehab1?
