5 days/week Pride Health is hiring a Travel Physical Therapist (PT) to support our client's healthcare facility in Huntingdon, Pennsylvania.

$50/hr

Pay offered is based on experience, expertise, credentialing, and education.

Duties:

Provide skilled physical therapy services to patients in their homes.

Evaluate patients, develop individualized treatment plans, and monitor progress toward functional goals.

Complete all required clinical documentation, assessments, and OASIS documentation accurately and timely.

Collaborate with physicians, nurses, and interdisciplinary care teams to coordinate patient care.

Educate patients and caregivers on therapeutic exercises, mobility techniques, safety precautions, and home exercise programs.

Requirements:
Education:

Graduate of an accredited Physical Therapy program (DPT or PT degree) License(s): Active Physical Therapist (PT) license in Pennsylvania Certification(s): Current BLS Certification preferred

Years of Experience:

Minimum 1 year of Physical Therapy experience preferred

Home Health experience strongly preferred
