Schedule & Assignment Highlights:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Approximately 2532 points per week (typically 57 patient visits per day) Field-based care, with daily travel in the assigned territory

Key Qualifications:

Active Physical Therapist (PT) license in Pennsylvania, in good standing At least one year of recent field-based Home Health PT experience OASIS assessment experience required Proficiency in Home Care Home Base (HCHB) preferred Strong clinical reasoning and patient assessment skills Excellent time management, organizational, and communication abilities Ability to work independently while ensuring quality, patient-centered care Reliable transportation, valid drivers license, and current auto insurance

What Youll Do:

Conduct comprehensive physical therapy assessments, including OASIS when applicable Develop and execute tailored treatment plans focusing on mobility, strength, balance, and functional independence Implement effective interventions such as gait and transfer training, therapeutic exercises, and fall prevention strategies Monitor and document patient progress using HCHB Collaborate with physicians and the interdisciplinary care team for coordinated service delivery Educate patients and caregivers on rehabilitation exercises, assistive devices, and home safety practices Participate in case conferences, interdisciplinary meetings, and ongoing professional development
