MIST Therapy Skilled Nursing Facility Wound Care:
Los Angeles Metro Area California Wound Healing Medical Group is seeking a full-time Physical Therapist to provide MIST Therapy (non-contact, low-frequency ultrasound) to residents in Skilled Nursing Facilities throughout the Los Angeles Metro area. This is not a traditional outpatient or home health physical therapy position. You will work within a specialized wound care program, treating residents with chronic, complex, and non-healing wounds while collaborating with providers and interdisciplinary facility teams. This role is ideal for a clinically independent Physical Therapist who is comfortable working across multiple facilities, managing an assigned caseload, and maintaining accurate, audit-ready documentation.
POSITION OVERVIEW
Full-time W-2 position Facility-based care within Skilled Nursing Facilities Los Angeles Metro service area Independent clinical practice with support from a collaborative wound care team Specialized training available in advanced wound care and MIST Therapy
KEY RESPONSIBILITIES
Clinical Care and Treatment Deliver MIST Therapy treatments in accordance with physician orders, clinical protocols, and applicable standards of care Perform focused wound assessments and evaluate healing progression, treatment response, and signs of deterioration or non-healing Apply therapy based on clinical indications, including pain management, contraindications to sharp debridement, deep tissue injuries, and other complex wound presentations Support provider-directed treatment plans and promptly escalate clinical concerns or significant changes in resident condition Educate residents, families, caregivers, and facility staff regarding treatment expectations and relevant wound care considerations Maintain the proper handling, cleaning, transportation, and readiness of MIST Therapy equipment Documentation and Compliance Complete accurate, timely, and defensible EMR documentation for all resident encounters Provide clear clinical assessments that support treatment decisions, outcomes, and continuity of care Ensure documentation supports medical necessity, billing requirements, and audit readiness Follow HIPAA, infection-control, facility, and regulatory standards Participate in quality assurance and compliance activities as needed Facility Operations and Care Coordination Manage an assigned facility caseload and maintain daily treatment schedules across SNF locations Meet established productivity expectations while maintaining quality, safety, and resident-centered care Communicate proactively regarding scheduling changes, resident availability, delays, or barriers to treatment Collaborate with physicians, nurse practitioners, nursing teams, rehabilitation staff, and facility leadership regarding treatment plans and resident progress Travel between assigned Skilled Nursing Facilities throughout the Los Angeles Metro area
REQUIRED QUALIFICATIONS
Active, unrestricted California Physical Therapist license in good standing Minimum of one year of clinical experience Current BLS/CPR certification Valid driver's license and reliable transportation Ability to travel between assigned facilities Ability to work independently in a facility-based clinical environment Strong communication, organizational, and documentation skills
PREFERRED QUALIFICATIONS
Wound care experience, particularly involving chronic, complex, or non-healing wounds Experience working in Skilled Nursing Facilities, post-acute care, long-term care, or another interdisciplinary clinical setting Experience with MIST Therapy or other advanced wound care modalities Proficiency with EMR documentation systems Familiarity with medical necessity and audit-focused documentation standards
