Responsibilities $5,000 Sign-On Bonus Available The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress. Qualifications Education Bachelor's Degree in a health-related field like biology, exercise science, or kinesiology Required and Graduate of an accredited program from a CAPTE-accredited school Required Work Experience Previous experience recommended Licenses and Certifications PT•Physical Therapist Upon Hire Required Must be currently licensed in Physical Therapy in the state of practice. Required CPR•Cardiac Pulmonary Resuscitation Upon Hire Required
