Job Description

Category Homehealth - PT Job Type Contract Take the next step in your healthcare career as a Physical Therapist specializing in home health. This position offers the opportunity to support adult patients managing chronic diseases, wound care, and rehabilitation needs in a dynamic contract role.
Schedule/Hours:
8-hour day shifts, Monday to
Friday Shift:
8:00 AM - 5:00 PM, 40 hours per week No weekend rotation or on-call requirements Must work holidays as scheduled
Key Qualifications:
Active State PT license and current BLS certification Bachelor's degree in Physical Therapy required Minimum 1 year of experience in physical therapy (home health strongly preferred) Experience with Homecare Homebase EMR (no exceptions) and use of OASIS-E assessment tool Valid driver's license and proof of insurance Strong background in wound care (wound vac, pleurx), IV management, assessment, and chronic disease education Skilled with adult and geriatric patients Ability to chart efficiently and communicate effectively in a home health environment
Role Responsibilities:
Complete home visits for an adult caseload (approximately 6 points per day) Provide therapy, disease management, and patient/family education Assess, document, and report patient progress using Homecare Homebase Collaborate with department leadership and interdisciplinary teams Participate in a 5-day orientation and ongoing department support
Additional Details:
Holiday coverage required as scheduled, with no floating or weekend rotations Samsung Note 9 provided for work use Wear grey scrubs, per therapy department guidelines Coverage area includes York and Lancaster counties; floating to other counties is rare Advance your professional journey by supporting patients where they're most comfortable—their homes.
